First, submit a Request to Pay a Service Provider (RTP) form on the My Family Services portal.

To do so, follow the directions below, or email or call Risa for assistance (admin@kerryschroeder.ca/778.655.0904).

• Log in to your account on the My Family Services portal 

• Click view details beside the child whose funds you’d like to use

• Click request to pay a service provider 

• Click add new provider

• Select business

• Business name: KS Counselling Inc.

• Phone number: 778.655.0904

• Address: 167 Creekstone Way SW, Calgary AB, T2X 4P9

• What type of service do you want to authorize this provider for?
  : select Intervention

• Click continue

• Start and end date: today’s date (or a date before your first session) until as far into the future as you’d like

• Total amount: the amount you’d like to allocate for counselling/coaching (this can be adjusted)

• What is the type of service being requested? Select Family Counselling/Therapy

• Rate per hour: max $250/hr (max $275/hr for informal autism screenings). This rate accounts for intake, 60-min, and 45-min sessions.

If submitting a Request to Pay Service Providers form and emailing it in or sending in via the portal messenger, it’s almost the same. You can just put KS Counselling Inc. as the agency, and check the “agency” box.
